Staff RN - Experienced Bedside - 4th Floor Med/Surg

About the Role

Coordinates professional nursing and individualized, family-centered care to patients and families through the use of the nursing process and clinical skills in accordance with standards and hospital policies and procedures. On-call responsibilities vary depending on the department. Varied schedules include weekend and holiday patterns.

Schedule

7p–7a, rotating weekends and holidays (36 hours per week).

Responsibilities

• Conducts individualized age-specific patient/family assessments according to defined unit scope and nursing practice standards, including collaborative problems (CPGs), nursing diagnoses (NDs), or protocols.
• Performs admission assessments, collecting and documenting data related to patient’s individualized care, including developmental, cognitive, cultural, psychosocial, spiritual, and educational/family influences.
• Demonstrates age-specific competency in patient assessments, interpreting information based on patient needs, growth and development, and treatment requirements.
• Assesses patient/family need for education and uses an individualized approach in delivery style.
• Identifies nursing diagnoses, collaborative problems, and/or protocols for each patient at the time of admission.
• Performs shift assessments/reassessments based on unit standards, CPGs/NDs, protocols, and changes in patient condition, communicating changes to appropriate personnel (MD, Charge RN, family).
• Develops and revises individualized patient/family plan of care using CPGs/NDs, practice guidelines, and protocols to meet identified needs, including collaboration with the family and healthcare team.
• Implements and evaluates nursing actions in accordance with the individualized patient/family plan of care and patient condition/needs.
• Oversees coordination, implementation, evaluation, and revision of the nursing and interdisciplinary plan of care/goals.
• Uses appropriate resources (CNS/Educator, Director, MD, etc.) to ensure clinical outcomes are achieved.
• Provides and/or coordinates patient/family education according to identified care needs, considering readiness to learn.
• Documents accurate assessment/evaluation, modification, and patient’s response to therapy, changes in condition, and therapeutic interventions/procedures in the medical record.
• Completes documentation within established timeframes, including admission assessments, daily flowsheets, care plans, medication records, and discharge instructions.
• Ensures discharge instructions clearly state patient care needs after discharge.
• Regular attendance at work is required.
• Performs physical requirements as described in the job’s physical requirements section.
• Assists in arranging and coordinating appointments prior to the delivery process.
• Mentors student nurse observation experiences and acts as a relief preceptor for new staff.
• Professionally seeks opportunities to assist teammates during shifts as needed.

Cath Lab-Specific Responsibilities

• Schedules cardiac catheterization and electrophysiology procedures with patient families.
• Works closely with OR supervisors, OR Manager, Cath Lab staff, and Director of Anesthesia.
• Obtains permits for catheterization procedures after educating patient/family about the procedure and related complications.
• Collects and documents data related to patient’s response to procedures and changes in condition.
• Circulates in procedures following AORN guidelines/standards.
• Assists at bedside in preparing patients for procedures.
• Applies critical thinking during catheterization procedures, prioritizing interventions based on continual assessment.
• Coordinates with referring physicians and other operating physicians to assess patient/family care issues in scheduling procedures.
• Accompanies patients/families to rooms to facilitate patient flow.

Requirements

• Associate's Degree from an accredited school of nursing Required or Bachelor's Degree from an accredited school of nursing Required.
• Minimum 1 year of RN experience Required.
• Experience with pediatric patients Preferred.
• Previous experience with EPIC Preferred.

Skills and Abilities

• Ability to perform a variety of duties, often changing tasks of a different nature without loss of efficiency or composure.
• Ability to perform under stressful conditions when confronted with multiple requests, demonstrating prioritization skills.

Licenses and Certifications

• RN - Registered Nurse - State Licensure and/or Compact State Licensure (current and valid in the state of Nebraska) Upon Hire Required.
• BLS - Basic Life Support through the American Heart Association Upon Hire Required.
• PALS - Pediatric Advanced Life Support (except NICU) within 180 Days Required.
• NRP - Neonatal Resuscitation (NICU only) within 180 Days Required.
• ACLS within 9 months of hire (Cath Lab and ED only) Required.
• ENPC - Emergency Nursing Pediatric Certification (ED only) within 1 Year Required.
• TNCC - Trauma Nursing Core Course (ED only) within 1 Year Required.
• For RNs in Radiology Nursing, training and completion of all required competencies are mandatory.
