About the role

Provides leadership in optimal product preparation and dosing of medications, and provision of patient-specific drug information to include patient-specific dosing. Aids in the prevention and reporting of medication errors and adverse drug reactions according to policy. Supports quality relationships with health care professionals, families, and patients; and the pharmacist is actively involved in being further educated and providing education in all appropriate ways.

Position will be located at the Medical Center East Pharmacy, which is a 24-hour outpatient/discharge pharmacy. Hours are Monday - Friday 9:30am - 6:00pm with every 8th weekend rotation (both Saturday and Sunday) and holiday rotation.

Responsibilities

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate, and cost-effective use of medications.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Prepares and dispenses medications, provides drug information, and manages medication therapy in accordance with federal, state, and local regulations and hospital policy.
  • Supervises the activities of assigned support personnel and verifies the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or healthcare providers.
  • Mentors and trains others by setting an example, giving instruction, and providing leadership to raise the level of performance.
  • Conducts medication management, including determining the clinical appropriateness of medications, dosage, potential adverse effects, and monitoring laboratory results related to medication therapy.
  • Provides drug information to healthcare professionals, patients, and their families.

Requirements

  • Pharmacist License (LIC-Pharmacist License - Tennessee).
  • Bachelor’s degree (required).
  • Less than 1 year of relevant work experience.

Skills

  • Pharmaceutical Computer Literacy (Novice): Proficient understanding of computers or successful completion of training for applications supporting medication use processes. Capable of using fundamental features of pharmaceutical computer applications to support patient care issues of moderate difficulty.
  • Pharmaceutical Drug Preparation, Dispensing & Distribution (Intermediate): Demonstrates proficiency in drug preparation, dispensing, and distribution in practical applications of a difficult nature. Able to review, prepare, and assist in dispensing prescribed medications, including maintaining sterility of parenteral medications where applicable.
  • Pharmaceutical Drug Preparation & Dispensing (Intermediate): Proficient in drug preparation and dispensing, including aseptic technique for compounding sterile preparations (CSPs) to minimize contamination risk.
  • Medication Management (Intermediate): Proficient in medication management, including determining clinical appropriateness, dosage, contraindications, and monitoring laboratory results related to medication therapy.
  • Mentoring & Coaching (Novice): Constructively trains and mentors others by providing positive direction and feedback. Removes roadblocks to success and avoids judgments.
