Staff Pharmacist

Responsibilities

• Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions.
• Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug-drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency.
• Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records.

Qualifications

• Education: Required: Pharm. D. or BS in Pharmacy
• Experience: Preferred: Hospital experience
• Licenses: Current registered pharmacist license in the State of Florida
• Certifications: Required: BLS within 30 days of hire (ACLS and/or PALS if code attendance required); Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.
