About the role
Crouse’s Pharmacy team is hiring a Staff Pharmacist.
Schedule
- Full-Time, Day/Evenings/Nights
- 7:00am–3:30pm, 3:00pm–11:30pm, 8:30pm–7:00am
- Rotating weekends, holidays, and other shifts as needed
Pay
$79.00/hour plus applicable shift differential.
Responsibilities
- Completely evaluates all medication orders against clinically appropriate, patient-specific parameters.
- Resolves any identified issues utilizing hospital-approved guidelines or with the appropriate provider; documents all interventions.
- Initiates substitution of a formulary medication for a non-formulary medication when clinically appropriate.
- Appropriately documents all clinical activities.
- Reviews all orders for IV contrast as provided by Radiology.
- Efficiently utilizes both hospital-based and pharmacy-based computer systems.
- Assures that all products are appropriately labeled prior to dispensing.
- Participates in maintaining a working stock of medications and supplies.
- Ensures proper storage of medications.
- Ensures that a patient, nursing unit, or department is charged for all medications dispensed/materials utilized.
- Dispenses a 24-hour supply of unit-of-use medications to individual patients for all new orders.
- Checks unit-dose carts filled by pharmacy assistants.
- Checks prepackaged unit-dose medications prepared by pharmacy assistants.
Requirements
- Current New York State Pharmacy License
- Written and oral communication skills and interpersonal skills to deal effectively with other healthcare professionals and hospital employees
