Position Summary

Success in this role will perform general Pharmacist duties, including but not limited to: preparing medications, completing final check on filled medications, reviewing and approving data entry of prescriptions, monitoring patient drug therapies, and conducting drug utilization reviews.

Responsibilities

  • Prepares and/or oversees the preparation and dispensing of prescription medications to patients
  • Makes decisions regarding generic substitution of prescribed pharmaceuticals
  • Makes outbound or takes inbound telephone calls to assist in various aspects of patient care and/or product distribution to/from practitioners, members, and pharmacies
  • Receives physician prescription orders and evaluates the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration, and stability
  • Supervises the filling, receiving, and dispensing activities involved in providing finished product to the patient
  • Ensures that the patient chart is complete for allergies, current and past medical history, and concurrent medications (including over-the-counter or home remedy medications)
  • Provides patient counseling, education, and monitoring
  • Affirms safety of patients and personnel by exercising good pharmacy practice in the process of drug therapy instructions to the patient
  • Determines remaining doses on hand to manage inventory and avoid waste
  • Assists the physician, nurse, and patient with clinical and technical support
  • Documents all communication with physicians, nurses, and patients in the patient chart
  • Affords assistance to the Pharmacist-In-Charge in implementing new procedures
  • Affords assistance and supervision to Pharmacy Technicians and other pharmacy personnel
  • Participates in Performance Improvement Projects
  • Provides in-service programs to clinicians and pharmacy personnel on any aspect of drug therapy
  • Reviews literature and other materials pertinent to the practice of pharmacy
  • Completes all competency/skills assessment requirements
  • Participates in on-call schedule if business needs warrant
  • Performs other tasks as determined by the Pharmacist-In-Charge and pharmacy leadership
  • Provides requested information, answers questions, and responds to requests from patients and/or medical practitioners in person or by telephone related to drug indications/contraindications, dosage, drug interactions, and side effects; provides other drug and pharmaceutical information as appropriate
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, HIPAA, and Private Insurance regulations and requirements
  • Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration
  • Maintains all required pharmacist state licensure resident and non-resident

Qualifications

  • Must be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
  • Current Pharmacy license in applicable state, and additional state licenses (CA, FL, VA) preferred
  • Licenses must be maintained and in good standing
  • Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration
  • Knowledge of patient care charts and patient histories
  • Knowledge of quality, safety, and/or infection control standards
  • Knowledge of related accreditation and certification requirements
  • Ability to communicate technical information to non-technical personnel
  • Knowledge of policies and regulations pertaining to the disbursement of pharmaceutical drugs
  • Knowledge of clinical operations and procedures
  • Skills in performing basic patient assessments and referrals
  • Strong interpersonal relations skills
  • Excellent planning, organization, time management, and execution skills
