Pay: Starting at $29.00/hour, based on date of state certification.

About the role

At Community First Solutions, our nursing team takes a resident-first approach—ensuring wellness plans are guided by each individual’s needs and goals. As a Staff Nurse LPN/RN, you’ll work side-by-side with our caregivers, providing direct bedside care, administering medications and treatments, and collaborating with physicians and the interdisciplinary team to support residents’ health and well-being. You’ll also guide and mentor caregivers, ensuring a safe, supportive, and person-centered environment. Your leadership at the bedside helps set the tone for quality, compassion, and teamwork—making Community First Solutions a place where both residents and staff thrive.

Responsibilities

  • Institutes appropriate nursing intervention to assist with a resident’s condition and/or prevent complication.
  • Performs resident rounds to assess physical and emotional status, implementing any required nursing interventions.
  • Responds to resident and family concerns and ensures that each concern is documented, and a resolution is initiated or communicated to appropriate staff.
  • Coordinates admission, discharge and transfer of residents.
  • Prepares, administers and documents medications when given.
  • Ensure that nurse on duty is informed of all previous shift reports and keeps charge nurse and other staff members informed about resident status.
  • Consults with licensed or registered nurse, physician or licensed independent practitioner when appropriate.
  • Clarifies any order or treatment regimen believed to be inaccurate, ineffective, or contraindicated by consulting with the appropriate licensed provider.
  • All other duties as assigned and/or appropriate to the position.

Requirements

  • Education – Graduate from an Ohio Board of Nursing approved nursing program or school of nursing
  • Licenses – Must have an active LPN or RN license in the state of Ohio, CPR certified
  • Specialized Knowledge, Skills, or Abilities
    • Excellent bedside manner
    • Strong organizational skills and attention to detail
    • Excellent written and verbal communication skills
    • Experience working with an electronic health record system
