SSVF Case Manager – DFW Area

Responsibilities

• Conduct intakes and housing navigation
• Develop Housing Stability Plans
• Provide case management to assist veterans with housing stability goals
• Follow guidance and regulations of SSVF to provide financial assistance
• Work with the Health Care Navigator and the Employment Specialist to ensure participation with additional services
• Create and maintain positive working relationships with properties in the area
• Participate and represent the program in community team meetings, case conferences, and other staff meetings
• Timely and accurate documentation of services and activities in HMIS and the agency’s client records

Requirements

• Bachelor’s degree in Social Work or related field
• 2 years of experience providing case management in the human service field
• Experience working with people experiencing homelessness and/or veterans preferred
• Reliable transportation, current Texas driver’s license, and evidence of auto liability insurance
