SSVF Carolinas Case Manager- Fayetteveille (68977)

Responsibilities

• Coordinate outreach efforts for the SSVF Program
• Screen potential program participants for program eligibility
• Conduct housing and comprehensive supportive services assessments
• Work with persons served, family members, collaborating housing agencies and professionals to revise service plans
• Provide or arrange for housing support and wraparound services
• Communicate effectively with clients and service team to address issues
• Ensure appropriate use of and documentation of temporary financial assistance
• Ensure participant category reassessments are completed according to specific SSVF requirements
• Participate in program and professional supervision

Qualifications

• A bachelor’s degree in counseling psychology, rehabilitative counseling, or related human service field (or comparable experience)
• Experience working with homeless individuals and families, including veterans (plus)

Other

• Requires driving a personal vehicle for company business
• Must have valid drivers license and liability coverage
• May require acceptable criminal background check and OIG Exclusion check
• Must pass negative drug screening
• Must have CPR/First Aid Certification
• Must be able to operate a motor vehicle to transport oneself, consumers, and program supplies
• Must be able to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and perform basic and light home maintenance activities
• Must be able to operate office equipment requiring continuous or repetitive hand/arm movements
