Sr. Clinical Consultant Social Worker/Intake Clinician, Behavioral Health (LISW/LPCC)

About the Role

This position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at an assigned hospital. The Social Worker MSW, LISW provides services to patients, families, and significant others, including biopsychosocial assessments, psychiatric diagnoses, treatment planning, therapy sessions (individual, couple, family, or group), and discharge planning. This role supports and enhances biopsychosocial functioning and receives referrals for at-risk populations from interdisciplinary team members.

The clinician practices independently as permitted by Ohio licensure and may perform advanced functions such as staff education, training, mentoring, project development, and administrative or supervisory responsibilities. This position may also oversee select social service functions, including scheduling, case assignments, and supervising social work staff and master’s level interns. The role requires the ability to act quickly and work in high-stress situations.

Responsibilities

• Advanced Clinical Assessment (30%)
  • Conducts patient/family/significant other interviews, chart reviews, and professional collaboration to assess biopsychosocial functioning.
  • Evaluates developmental and age-specific needs, self-care ability, dependent care, basic custodial/financial needs, social/emotional relationships, and treatment compliance.
  • Develops care plans, including discharge plans, aligned with medical care.
  • Identifies and reports cases of abuse, neglect, or exploitation to mandated authorities.
  • Provides crisis intervention, information, referrals, and linkages to community resources.
• Advanced Intervention Management (30%)
  • Develops intervention plans based on assessments to resolve or support biopsychosocial functioning.
  • Uses clinical intervention skills, including treatment planning, continuum of care planning, supportive counseling, crisis intervention, and educational counseling.
  • Collaborates substantially with interdisciplinary teams for holistic care planning.
• Advanced Population Management (25%)
  • Coordinates clinical and financial outcomes for an assigned patient population.
  • Works with interdisciplinary staff and external organizations to manage timely, cost-effective care.
  • Monitors service quality and ensures care management within established parameters.
  • Liaises with payers to manage post-discharge financial resources and treatment progress.
  • Ensures completeness of treatment plans and timely reporting.
  • Helps patients coordinate care for quality of life while managing costs.
  • Assists families in accessing knowledge and resources.
• Documentation and Regulatory Compliance (10%)
  • Thoroughly documents assessments and interventions in medical records and information systems.
  • Complies with internal and external documentation requirements.
• Community Resource Awareness (5%)
  • Maintains awareness of current hospital and community services.
  • Provides information and facilitates linkages between services and patients.
• Performs additional functions as appropriate to the position.

Requirements

• Master’s Degree in Social Work (Required).
• Licensed Independent Social Worker (LISW) – Social Work Certification and Licensure Board or Licensed Professional Clinical Counselor (LPCC) – American Counseling Association.

Schedule

Day shift, scheduled for 40 hours per week.
