About the role
The Spiritual Counselor at Riverside Hospice Agencies provides emotional and spiritual support to patients, families, and caregivers. They participate as a member of the interdisciplinary team and support the agency's mission and philosophy.
Responsibilities
- Provides assessment as indicated and appropriate after admission, supporting the patient/family's wishes and needs.
- Completes required continuing education classes and in-service programs as assigned, staying current in clinical knowledge.
- Participates in inter-disciplinary team meetings and discussions regarding quality and outcomes.
- Develops an individualized spiritual/pastoral plan of care, collaborating with local clergy if desired by the patient, and providing spiritual support as needed.
- Recognizes and responds to patient care/condition variances, informing the physician and other personnel of changes and outcomes.
- Communicates information using current processes and technology, notifying the physician and team promptly according to CHAP standards.
- Strives to exceed customer/patient expectations through flexibility and sensitivity.
- Documents counseling provided in the patient's clinical record.
- Aids in the development and updating of the patient's plan of care.
- Ensures individual productivity measures are satisfactory based on FTE/Status and average number of assigned patients in the case load.
- Buils trusting collaborative relationships with team members.
- Collaborates with other spiritual coordinators/counselors.
Requirements
- Masters Degree in Divinity or Theology (Required)
- Related Hospice experience (Preferred)
- ACPE Educator Certification from the Association for Clinical Pastoral Education within 2 years (Preferred)
- American Council of Pastoral Education certification upon hire (Required)
- Valid Drivers License (Required)
- Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job.
Qualifications
- Education: Masters Degree, Divinity or Theology (Required)
- Experience: Related Hospice experience (Preferred)
- Licenses and Certifications: ACPE Educator Certification from the Association for Clinical Pastoral Education within 2 years (Preferred), American Council of Pastoral Education certification upon hire (Required)
- Other Requirements: Valid Drivers License (Required), Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job.
