Primary Functions
- Aids in the admission of the client to the Agency by conducting initial and ongoing assessments, including OASIS evaluations.
- Collects and interprets medical histories and discusses necessary changes with physicians.
- Develops and implements the interdisciplinary client care plan focusing on speech therapy.
- Provides skilled speech pathology services as directed by the physician, evaluating the client's level of functioning and hearing.
- Creates the plan of treatment in collaboration with the physician and follows up with progress notes.
- Schedules out-of-home services as needed and communicates with other team members about speech pathology techniques.
- Supervises Home Health Aides every two weeks when Speech Therapy is the sole professional discipline involved.
- Involves in discharge activities, completing the Speech Therapy discharge summary according to Agency policies.
Qualifications
- Master’s degree or equivalent from an accredited program in Speech Therapy, or equivalent combination of education and experience in Speech Therapy.
- At least 2 years of relevant Speech Therapy experience.
- Registered Speech Therapist in the State of Florida.
- CPR certification required.
