About the Role

The Speech Therapist in Home Health assesses, evaluates, and treats patients with speech, language, swallowing, and cognitive impairments. They develop and implement individualized treatment plans, monitor patient progress, and collaborate with healthcare team members.

Responsibilities

• Provide speech therapy services within legal scope and according to the care plan
• Evaluate patient function using diagnostic tests and assist in developing therapy plans
• Treat patients to improve communication skills and functional abilities
• Observe, document, and report patient responses and condition changes
• Educate patients, families, caregivers, and team members on therapy techniques

Requirements

• Licensed in Speech Therapy in the state of residence
• Current CPR certification
• Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit
