Role Overview: Provides speech therapy services in a home setting, evaluating patients, developing individualized treatment plans, and delivering direct therapy to address speech, language, cognition, voice, and dysphagia deficits. Key Responsibilities Assess patients through diagnostic assessments and interpret results to determine intervention needs.Develop, implement, and modify personalized treatment programs based on ongoing evaluations.Deliver therapy tailored to patients' physical, emotional, and cognitive capabilities, coordinating with other healthcare providers.Document evaluations, treatment plans, progress notes, and communicate effectively with patients, families, and team members.Participate in program development and supervise clinical internships with students. Qualifications & Skills CCC-SLP from ASHA or current supervised clinical fellowship with certification within one year, or active state licensure.Ability to work flexible hours, communicate effectively, and maintain confidentiality.Preferred 1-3 years of experience in various healthcare settings such as hospitals, SNFs, outpatient, or home health.
