Speech Language Pathologist (SLP) - Skilled Nursing (SNF) - PRN

Position Summary

Concept Rehab is seeking a PRN Speech-Language Pathologist (SLP) to join our rehabilitation team at a skilled nursing facility in Johnston, Iowa. This role offers exceptional flexibility for clinicians seeking supplemental income, schedule control, or work-life balance while delivering high-quality speech therapy services.

Key Responsibilities

• Evaluate residents and establish individualized treatment plans.
• Provide speech, language, cognitive, communication, and swallowing therapy services.
• Complete accurate and timely documentation, progress notes, and billing.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Participate in care planning and discharge planning meetings.
• Educate residents, caregivers, and facility staff on treatment strategies and maintenance programs.
• Maintain compliance with all federal, state, facility, and company guidelines.
• Support a positive, patient-centered rehabilitation environment.

Qualifications

• Master's Degree in Speech-Language Pathology.
• Current ASHA Certificate of Clinical Competence (CCC-SLP) preferred.
• Current Iowa Speech-Language Pathology license or eligibility to obtain licensure.
• Experience in skilled nursing, long-term care, rehabilitation, or geriatric settings preferred.
• Strong communication, documentation, and clinical decision-making skills.
