About the Role

Provide speech and language therapy services to patients with communication, speech, and swallowing needs under physician direction. Develop and implement individualized care plans, conduct assessments, and evaluate treatment outcomes.

Responsibilities

  • Communicate with physicians, team members, patients, and families, documenting care accurately and timely.
  • Participate in interdisciplinary team meetings, quality improvement programs, and staff education.
  • Supervise home health aides when appropriate and ensure patient rights and safety are maintained.
  • Promote positive relationships, adhere to ethical standards, and contribute to community education to support agency growth.
  • Adhere to infection control standards and participate in ongoing training.

Requirements

  • Bachelor’s degree in Speech Language Pathology required; Master’s preferred.
  • Minimum of one year experience in a home health setting.
  • Active, unencumbered Speech Language Pathologist license.
  • Strong communication, analytical, and documentation skills.
  • Ability to perform physical tasks, including lifting up to 50 pounds, and work in a healthcare environment with exposure to biological hazards.
