Speech Language Pathologist (SLP) In House SNF

About the role

Evolve Therapy Services is seeking a compassionate Speech Language Pathologist to provide in-house services at a Skilled Nursing Facility in Freeport, IL. This PRN position assists in planning, organizing, developing, and directing Speech Language Pathology services in accordance with federal, state, and local standards to ensure the highest degree of quality resident care.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate, following established standards, department procedures, and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans including target lists, short-term, and long-term goals.
• Instruct patients, families, and staff in speech-language procedures to support therapeutic remediation and goals in daily activities.
• Complete compliant and medically necessary documentation in NetHealth EMR systems, adhering to CMS expectations.
• Oversee and implement proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.

Requirements

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in the state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
