Responsibilities

  • Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
  • Develop a plan of care in consultation with the physician and other care team members.
  • Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
  • Instruct patients and families on using special educational strategies and techniques to improve sensory-motor development, perception, memory, language, and cognition.
  • Prepares clinical and progress notes of services provided, patient activities and condition and submit in a timely manner.
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Requirements

  • Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
  • Active Florida SLP license.
  • Minimum of 1 year of experience as a licensed Speech Language Pathologist (SLP).
  • Home health care experience preferred.
  • Strong clinical assessment, communication, and patient-education skills.

Qualifications
