Speech Language Pathologist (SLP) - Home Health

Responsibilities

• Evaluate patients' communication, hearing, and swallowing functions to develop personalized care plans.
• Perform comprehensive assessments and select therapeutic activities to improve language, speech, voice, and communication strategies.
• Monitor patient progress, modify treatment plans, and document interventions accurately.
• Educate patients, families, and caregivers to support recovery and discharge planning.
• Collaborate with healthcare team members to coordinate care and ensure optimal outcomes.
• Visually and auditorily assess patient status to inform treatment decisions.

Qualifications & Skills

• Master’s or doctoral degree in Speech Language Pathology or two years of recent relevant experience.
• Ability to evaluate and develop treatment plans for communication, hearing, and swallowing issues.
• Strong communication skills for patient and team collaboration.
• Commitment to providing compassionate, patient-centered care.
