Speech Language Pathologist (In House Therapy Team) PRN

Evolve Therapy Services is a therapy management organization hiring compassionate therapists for a partner skilled nursing facility in Oconto, WI. This is a PRN Speech Language Pathologist position.

About the role

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Administrator, to assure the highest degree of quality resident care is always maintained.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures, and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instruct patients, families, and staff in appropriate speech-language procedures to allow carryover of therapeutic remediation and goals in daily activities.
• Complete compliant and medically necessary documentation per CMS expectations in NetHealth EMR systems.
• Oversee and implement proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols to ensure residents' needs are met timely and with clinical excellence.

Requirements

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice (must have).
• Ongoing continuing education and professional development to maintain current licensure and certification.
