Speech Language Pathologist - Home Health Visits

About the Role

We are seeking an experienced Speech Language Pathologist (SLP) for home health visits, serving adult and geriatric patients in Northumberland County.

Responsibilities

• Perform home evaluations to assess, diagnose, and treat speech, language, communication, and swallowing difficulties.
• Develop, implement, and revise individualized treatment plans based on evaluations and physician directives.
• Educate patients, families, and caregivers in rehabilitative activities to promote health, safety, and independence.
• Accurately document observations, interventions, and assessments utilizing electronic medical records.
• Collaborate with a multidisciplinary healthcare team to coordinate patient care.

Requirements

• Current Pennsylvania licensure as a Speech Language Pathologist.
• Certificate of Clinical Competence from ASHA or equivalent education and experience, with ongoing certification process.
• Prior experience with electronic medical records preferred.
• Strong communication, assessment, and documentation skills.
