About the Role

Speech Language Pathologist (SLP) providing home health visits to adult and geriatric patients in designated areas.

Responsibilities

• Conduct evaluations, diagnose speech, language, communication, and swallowing issues, and develop personalized treatment plans.
• Deliver therapeutic interventions and educate patients and families.
• Document care activities using electronic records.
• Participate in multidisciplinary care planning.
• Ensure patient safety and independence.
• Perform home visits, assess patient needs, implement and revise therapy plans.
• Collaborate with healthcare teams and maintain accurate documentation to support patient care and progress.

Qualifications & Skills

• Current Vermont SLP license.
• Certification from the American Speech-Language-Hearing Association or equivalent qualifications.
• Experience with electronic medical records preferred.
• Strong assessment, communication, and documentation skills.
