About the role
We are seeking an experienced Speech Language Pathologist (SLP) to provide home health visits in designated areas.
Responsibilities
- Perform thorough evaluations and diagnose speech, language, communication, and swallowing issues.
- Develop and revise treatment plans tailored to patient needs.
- Instruct patients and their families on rehabilitative care to promote health, safety, and independence.
- Accurately document observations and interventions using electronic medical records.
Requirements
- Current Vermont Speech Language Pathologist license.
- Certification from the American Speech and Hearing Association (ASHA) or equivalent.
- Experience with electronic medical records preferred.
Skills
- Strong clinical assessment abilities.
