Speech Language Pathologist

About the Role

Wake Robin is seeking an innovative, skilled, and resident-centered Speech Language Pathologist to join our Rehabilitation Services team. Here, therapy is about more than treating a diagnosis – it’s about helping residents stay connected to the people they love, maintain their independence and continue living life on their own terms. In this role, you’ll partner closely with nurses, therapists, physicians, residents, and families to assess and treat communication, cognitive, and swallowing disorders. As an integral member of our interdisciplinary team, you’ll develop individualized therapy programs that promote function, confidence, and quality of life, with goals that are meaningful to each resident. Responsibilities

• Resident Assessment & Treatment
  • Evaluate residents' speech, language, cognitive-communication, and swallowing abilities.
  • Develop and implement individualized treatment plans based on physician orders, assessments, and resident goals.
  • Provide therapeutic interventions related to communication, cognition, memory, executive functioning, speech production, fluency, and swallowing.
  • Recommend and implement compensatory strategies, diet modifications, and adaptive approaches to support resident safety and independence.
  • Monitor resident progress and modify treatment plans as needed in collaboration with the interdisciplinary care team.
  • Educate residents, families, and caregivers on communication strategies, swallowing precautions, and therapeutic techniques to support ongoing success.
• Collaborative Care & Clinical Excellence
  • Partner closely with nursing, rehabilitation, and medical teams to coordinate resident-centered care plans.
  • Participate in care conferences and interdisciplinary meetings to ensure effective communication and continuity of care.
  • Deliver therapy using a holistic approach that incorporates each resident's lifestyle, interests, abilities, and personal goals.
  • Utilize evidence-based practices and remain informed on current innovations and best practices in speech-language pathology and geriatric rehabilitation.
  • Support quality improvement initiatives and contribute to ongoing program development efforts.
• Documentation & Compliance
  • Complete timely, accurate clinical documentation in accordance with professional, regulatory, and organizational standards.
  • Develop measurable treatment goals utilizing standardized assessments and clinical judgment.
  • Ensure compliance with CMS requirements, billing documentation, and departmental procedures.
  • Maintain accurate records of resident progress, treatment plans, and outcomes.
• Equipment & Program Support
  • Recommend, train, and support the use of adaptive equipment and technologies related to communication, cognition, and swallowing.
  • Provide guidance to residents and team members regarding safe feeding techniques, swallowing strategies, and therapeutic equipment.
  • Assist with staff education related to communication disorders, cognition, swallowing, and best practices in resident care.
  • Contribute to the continued growth and effectiveness of rehabilitation programs through quality improvement and data-informed decision making.

Requirements

• Master's Degree in Communication, Sciences and Disorders with a CCC-SLP certification, licensed in Vermont.
• Experience working with older adults required.
• Strong clinical skills in assessment and treatment of communication, cognitive-communication, and swallowing disorders.
• Excellent communication, documentation, and interpersonal skills.
• Ability to collaborate effectively within an interdisciplinary team.
• Strong organizational and communication skills, with the ability to prioritize and manage time effectively.

Schedule

32 hours per week, worked across 4 days Monday–Friday, with flexibility in the specific days and hours worked, typically between 7:00 a.m. and 4:00 p.m., based on resident service needs.

Pay

Hiring range: $40 - $55/hour. Starting salary is determined based on candidate's experience, education, and relevant skills as well as internal equity considerations.
