Responsibilities
- Conduct thorough assessments of residents' speech, language, cognitive, and swallowing abilities.
- Develop customized intervention plans to address communication and swallowing difficulties.
- Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments, Section K, BIMS, PhQ2-9, and other clinical data as applicable.
- Administer speech therapy interventions, including exercises, communication strategies, and swallowing therapy.
- Maintain accurate and timely documentation of resident progress and interventions in Point of Service (PoS) care delivery and EMRs (Net Health/PCC), including evaluations, treatment plans, daily and progress notes, and HMO/Managed care updates.
- Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.
- Conduct remote check-ins through Remote Therapeutic Monitoring (RTM) with discharged patients transitioning to home or the next level of care.
- Collaborate with other rehab staff, such as Physical Therapists and Occupational Therapists, to provide holistic care for residents.
