Your Role

You will provide one-on-one care in patients’ homes, helping patients improve communication, cognitive skills, swallowing function, and confidence in daily living.

Key Responsibilities

• Conduct in-home speech therapy evaluations and follow-up visits.
• Develop and implement individualized treatment plans.
• Assess and treat speech, language, cognitive, voice, and swallowing disorders.
• Educate patients and caregivers on communication techniques and home exercises.
• Document visits using WellSky Kinnser EMR.
• Collaborate with physicians, RNs, and the interdisciplinary care team.

Qualifications

• Active Arizona Speech Language Pathologist license.
• Master’s Degree in Speech-Language Pathology or related accredited program.
• ASHA Certificate of Clinical Competence (CCC-SLP) preferred.
• CFY candidates welcome to apply.
• CPR certification (AHA) preferred.
• Minimum 1 year of clinical experience (home health preferred).
• Strong clinical judgment and time management.
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.
