About the role
We’re the nation’s largest rehabilitation private practice to make Geriatric House Calls™ in patient homes and senior living communities, with over 4,000 clinicians treating in 38 states plus D.C. and growing.Responsibilities
- Provide geriatric care services in patients' homes and senior living communities.
- Collaborate with interdisciplinary teams to develop and implement treatment plans.
- Assess patient needs and modify treatments as necessary.
- Document progress and communicate with referring physicians and other healthcare providers.
Requirements
- Current license to practice physical therapy in at least one state.
- Experience in geriatric care preferred.
- Strong communication and interpersonal skills.
- Ability to travel and work in various locations.
Qualifications
- Master's degree in Physical Therapy from an accredited program.
- Pass the national certification exam.
- Pass the state licensing exam.
Skills
- Excellent clinical judgment and problem-solving skills.
- Effective communication and collaboration abilities.
- Proficiency in Microsoft Office Suite.
