Oasis Therapy Services is seeking a Speech-Language Pathologist to provide rehabilitation services in a senior care and skilled nursing setting in Prospect Heights, Illinois. Employment type: Full-Time. EMR: Net Health.

Responsibilities

• Evaluate residents and develop individualized treatment plans with measurable goals.
• Assess and treat speech, language, cognitive-communication, voice, and swallowing disorders.
• Provide skilled therapy interventions to improve communication, swallowing, safety, and quality of life.
• Monitor residents' progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers on communication strategies and swallowing precautions.
• Collaborate with nursing, physicians, rehabilitation professionals, and the interdisciplinary care team.
• Complete discharge planning and provide recommendations for continued care.
• Maintain accurate and timely Net Health documentation.

Qualifications

• Active and current Illinois Speech-Language Pathologist license.
• Master's or doctoral degree in Speech-Language Pathology from an accredited program.
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, skilled nursing, dysphagia, or rehabilitation experience is preferred.
