We’re currently looking for someone with a social services or human services background to join our remote team, partnering with people in the Wisconsin IRIS Program and the TMG IRIS Consultant Agency.

About the role

As an IRIS Consultant (IC), you will build relationships with participants and help them navigate and get the most out of the Wisconsin IRIS program—a Medicaid long-term care option for older adults and people with disabilities. You will identify participants' long-term care goals and find creative ways to achieve those goals, connecting them to community resources and developing customized IRIS plans for employment, housing, health, safety, community membership, transportation, and lasting relationships.

While the position is home-based, you will have regularly scheduled in-person visits with IRIS participants in their homes and communities. ICs play an important role in helping people of various backgrounds and abilities live the lives they choose.

Responsibilities

  • Meet in person with IRIS participants a minimum of four times per year, including one required annual home visit.
  • Be available upon the request of the participant, as IRIS is a self-directed program.
  • Provide program orientation to new participants, covering rights, responsibilities, legal documents, employee paperwork, and responsible use of public dollars.
  • Explore a broad view of the participant's life, including goals, relationships, community connections, employment interests, Self-Directed Personal Care awareness, and back-up support plans.
  • Assist participants in identifying personal outcomes and ensure those outcomes are met while staying within their IRIS budget and program requirements.
  • Document all orientation and planning activities in the IRIS data system (WISITs) within 48 business hours of visits.
  • Research and share community resources and natural supports that align with individual participant outcomes.
  • Document progress and changes in the plan and data system when modifications are requested, including completing Budget Amendment or One-Time Expense paperwork as needed.
  • Educate participants on interpreting their monthly budget reports to ensure they operate within their budget.
  • Serve as a liaison between the Fiscal Employer Agency and the IRIS Consultant Agency, assisting with provider billing, support brokers, receipt tracking, worker payments, and mitigating risks or conflicts of interest.
  • Develop engaged and trusting relationships with participants, communicate program changes, and maintain confidentiality and HIPAA compliance.
  • Collaborate with other IRIS Consultant Agency staff to ensure successful implementation of participants' plans.
  • Attend in-person monthly team meetings and participate in weekly phone check-ins with supervisors, along with other duties as assigned.

Requirements

  • At least 2 years of experience in health care, preferably in care coordination, and at least 1 year of experience serving target groups of the IRIS program (adults with intellectual/physical disabilities or older adults), or an equivalent combination of relevant education and experience.
  • Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or four years of commensurate experience if no degree).
  • Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job-related travel.
  • Ability to develop positive and effective work relationships with coworkers, clients, participants, providers, regulatory agencies, and vendors.
  • Ability to work independently with minimal supervision and demonstrate self-motivation.
  • Demonstrated knowledge of long-term care programs and principles of self-determination.
  • Problem-solving, critical-thinking, and excellent time-management skills.
  • Ability to focus on multiple projects simultaneously, adapt to change, and work through challenging situations.
  • Comfortable working in various settings and adjusting your style to work with diverse populations and personalities.
  • Demonstrated knowledge of community resources and proactive, detail-oriented approach.
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office suite and applicable software programs.

Pay

Pay range: $19.84 - $38.69 per hour. Actual compensation may vary based on geographic location, work experience, education, and skill level.
