Specialist, IRIS Consulting

About the Role

We’re looking for someone with a social services or human services background to join our remote team as an IRIS Consultant (IC). In this role, you will partner with people in your community who are enrolled in the Wisconsin IRIS Program—a Medicaid long-term care option for older adults and people with disabilities. While your office will be home-based, you will have regularly scheduled in-person visits with IRIS participants in their homes and communities.

As an IC, you will build relationships with participants, help them navigate the IRIS program, and identify their long-term care goals. You’ll find creative ways to achieve those goals, connecting participants to community resources and developing customized IRIS plans for employment, housing, health, safety, community membership, transportation, and lasting relationships. No two days are alike, and you’ll play a key role in helping people of diverse backgrounds and abilities live the lives they choose.

Responsibilities

• Meet in person with IRIS participants a minimum of four times per year, including one required annual home visit.
• Be available upon participant request, as IRIS is a self-directed program.
• Provide program orientation to new participants, covering rights, responsibilities, legal documents, employee paperwork, and responsible use of public dollars.
• Explore participants’ life goals, relationships, community connections, employment interests, Self-Directed Personal Care awareness, and back-up support plans.
• Assist participants in identifying personal outcomes and ensure those outcomes are met while staying within their IRIS budget and DHS program requirements.
• Document all orientation and planning activities in the IRIS data system (WISITs) within 48 business hours of participant visits.
• Research and share community resources and natural supports that align with participants’ individual outcomes.
• Document progress and changes in the plan and data system when modifications are requested, including Budget Amendment or One-Time Expense paperwork as needed.
• Educate participants on reading and interpreting monthly budget reports to ensure they operate within their budget.
• Serve as a liaison between the Fiscal Employer Agency and the IRIS Consultant Agency, assisting with provider billing, support brokers, receipt tracking, worker payments, and risk mitigation.
• Develop trusting relationships with participants, communicate program changes effectively, and maintain confidentiality and HIPAA compliance.
• Collaborate with other IRIS Consultant Agency staff to ensure successful implementation of participants’ plans.
• Attend in-person monthly team meetings with other ICs and supervisors, along with weekly check-ins and other assigned duties.

Requirements

• At least 2 years of experience in health care, preferably in care coordination, and at least 1 year of experience serving target groups of the IRIS program (adults with intellectual/physical disabilities or older adults), or an equivalent combination of relevant education and experience.
• Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or four years of commensurate experience if no degree).
• Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job-related travel.
• Ability to develop positive and effective work relationships with coworkers, clients, participants, providers, regulatory agencies, and vendors.
• Ability to work independently with minimal supervision and demonstrate self-motivation.
• Demonstrated knowledge of long-term care programs and principles of self-determination.
• Strong problem-solving, critical-thinking, and time-management skills.
• Ability to focus on multiple projects simultaneously and adapt to change.
• Comfort working in diverse settings and adjusting your style to work with various populations and personalities.
• Knowledge of community resources and a proactive, detail-oriented approach.
• Excellent verbal and written communication skills.
• Proficiency in Microsoft Office suite and applicable software programs.

Pay

Pay Range: $19.84 - $38.69 per hour. Actual compensation may vary based on geographic location, work experience, education, and skill level.
