Essential Job Duties and Functions
- Aid patients in creating individual service plans and fostering person-centered planning
- Share personal recovery stories to inspire and support patients
- Support patients in developing mental health advance directives
- Assist patients with problem-solving and reintegrating into the community
- Inform patients about community resources and assist with accessing them
- Provide support and guidance on recognizing and managing crises
- Help patients build and maintain positive support networks
- Encourage self-help skills and informed decision-making
- Plan and facilitate activities to boost self-esteem and self-concept
- Attend treatment/recovery team meetings
- Document services provided
- Advocate for patients to facilitate positive changes in their recovery
- Support patients during medical visits and follow-up with providers
- Facilitate communication and coordinate services between providers and patients
- Monitor and track patients' compliance with care plan objectives
- Collaborate effectively with the team and establish positive relationships
- Attend weekly huddles and mini huddles
Qualifications
- Meet The Wright Center for Community Health and its affiliated entity The Wright Center for Graduate Medical Education EOS© People Analyzer Tool requirements
- Have experience working in the EOS® model (preferred)
- Be mission-oriented and represent the organization professionally
- Have a high school diploma or equivalent (GED)
- Hold an active certification as a Certified Peer Specialist from the Pennsylvania Certification Board (PCB)
- Possess a valid PA driver’s license
- Have current BLS Certification
- Have knowledge of community resources
- Have knowledge of mental health peer support services
- Understand the recovery process and its application to individuals with serious mental illness
- Be able to follow written and oral instructions
- Be able to maintain effective working relationships
- Be able to communicate effectively orally and in writing
- Have general computer knowledge and capability to use computers
- Be able to interact effectively with practitioners, the interdisciplinary healthcare team, community agencies, patients, and families
- Be able to work autonomously and be directly accountable for practice
- Be self-directed, flexible, and committed to the team vision
- Be a team player, willing to learn, accept and respect diversity without judgment, and demonstrate strong customer service values
