Special Programs Case Manager I (Licensed BH Clinician)

About the role

The Special Program Case Manager I role is based in Atlanta, GA and offers a hybrid schedule with in-office 1-2 days per week. The position requires a license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist and involves managing healthcare costs, developing care plans, coordinating with internal and external resources, and addressing social determinants of health.

Responsibilities

• Conducts assessments to identify individual needs.
• Develops comprehensive care plans to address identified needs.
• Serves as a point of contact for member access to appropriate care.
• Coordinates with various state agencies and internal teams.
• Maintains knowledge of the system of care philosophy and coordinates with designated populations and their families.
• Evaluates health needs and identifies applicable services and resources.
• Provides patient education, medication reconciliation, and assistance with follow-up care.

Requirements

• Requires MS/MA in social work, counseling, or a related behavioral health field or a degree in nursing with 3+ years of clinical experience in social work counseling.
• Active, current, and valid license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist.

Preferred Skills, Capabilities, And Experiences

• Experience working with specialty populations.
• Clinical social work experience with displaced, foster, or adopted children and their families.
