SOS Team Leader

About the role

RiseWell Community Services is looking for a Full-Time Team Leader to join the SOS Team in Brooklyn, NY.

Responsibilities

• Implement and maintain the program model.
• Oversee daily operations of the critical support team and program.
• Assist the team with community outreach, specifically on the streets and subways.
• Coordinate participants' needs before and after moves, including daily living skill enhancement, accompanying to appointments, advocacy, etc.
• Develop and maintain collaborative relationships with community partners, including the Department of Homeless Services, MTA, local law enforcement, community hospitals, existing outreach teams, and housing providers.

Requirements

• Master’s degree or higher in Social Work, Mental Health Counseling, Public Administration, Nursing, Public Health, Public Policy or related field.
• Minimum of 3 to 5 years post-masters work experience in the behavioral health and criminal justice field.
• Experience with target population preferred.
• Previous management experience preferred.
• Knowledge of NYC subway system.

Qualifications

• Fingerprinting, criminal record check, approval from NYS Office of Mental Health required.
