- Certifications: BLS-AHA (copy needed); CERT-ARDMS-RDMS(AB) (copy needed)
- Sonographer, OB/GYN Certifications: RDMS-OB-American Registry for Diagnostic Medical Sonography
- SSN Required
- DOB Required
- Work History: Should not be more than 90 days of gap in last 1 year
