What You’ll Do We are looking for a Social Worker to join our team within the After School Intensive Outpatient Program (IOP). As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient’s primary treatment team to ensure a cohesive and effective plan of care. Responsibilities And Accountabilities Assessment and Planning: Conduct thorough assessments, maintain the care plan, and lead groups to enhance patient understanding and engagement.Patient and Family Engagement: Maintain open lines of communication with patients and their support systems to involve them in treatment plan decisions and foster a collaborative approach to care.Ethical Advocacy: Deliver and advocate for patient-centered, culturally appropriate, and age-appropriate care, ensuring that all services align with ethical standards and best practices. Your expertise will be instrumental in guiding patients on their journey to recovery. What Makes You a Great Fit Master’s Degree in Social Work with licensure in the State of Minnesota required. Experience in healthcare setting preferredExperience using electronic health records preferredProficient in Microsoft Office Products including Word and OutlookAble to effectively and efficiently communicate in written and verbal formAbility to smoothly and rapidly shift between dissimilar tasksAbility to communicate well both verbally and in writingAbility to react quickly and calmly as part of a team to handle volatile and potentially emergency situationsAbility to make effective decisions while in stressful situations and circumstances Compensation for this role ranges from $26-$34 per hour depending on education and experience.
