About the Role

The social worker provides professional services to patients and families, facilitating the use of individual, public, and private resources to achieve optimal functioning. Core services include:

  • Assessment of psychosocial functioning and capacities
  • Counseling for care planning and decision-making
  • Social work treatment and community resource planning

Social workers collaborate with multiple disciplines, programs, departments, and organizations to fulfill these duties.

Responsibilities

  • Conducts psychosocial assessments for adolescent, adult, geriatric, neonatal, infant, and pediatric patients
  • Determines the inter-relatedness of psychosocial factors and health status/disease process
  • Identifies emotional, social, and financial needs of patient/family and provides assistance/referrals as appropriate
  • Develops and follows up on appropriate treatment plans with the discharge planning team
  • Acts as an expert resource in social work, assisting with complex cases
  • Provides emotional support to patients and families, helping them cope with illness, disability, and challenging situations
  • Facilitates patient/family support groups (e.g., cancer, stroke, AIDS)
  • Participates in interdisciplinary discharge planning, conducting proactive assessments and documenting plans
  • Implements discharge plans, including setting up home health, transfers to other facilities, and initial nursing home placements
  • Assists in quality improvement initiatives, monitoring and trending hospital readmissions
  • Manages high-risk referrals, including discharge planning for newly diagnosed cancer patients, suspected abuse cases, adoptions, high-risk mothers, fetal demise, long-term acute placements, ER and psych transfers, HIV/AIDS, and hospice patients
  • Assists case management in transitioning patients to more appropriate levels of care
  • Promotes effective communication and ensures continuity of care through clear, positive interactions with patients, families, hospital staff, nurses, physicians, and the case management team
  • Maintains a supportive, collaborative relationship with the care team
  • Consistently performs all social service functions to meet department volume and quality standards
  • Communicates workload status to supervisor daily
  • Performs other duties as assigned
  • Maintains regular and reliable attendance
  • Complies with all policies and standards

Requirements

  • Master’s degree in Social Work or Psychology
  • 2–4 years of experience in individual, group, and family therapies
  • LCSW – Licensed Clinical Social Worker (required)
  • BCLS – Basic Life Support (obtained within 7 days of employment)
