Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Affirmatively ensure that VITAS patients have their affairs in order regarding finances, funeral arrangements, and other matters.
• Navigate the grieving process for patients' loved ones, offering guidance and support.
• Engage in activities that enhance the quality of life for patients and their families, such as fulfilling end-of-life wishes or simply being present for patients at their bedside.

Requirements

• Ability to travel as needed.
• Thorough knowledge of community resources or willingness to acquire this knowledge.
• Knowledge of key psychosocial issues affecting terminally ill patients and their families, along with the ability to implement effective interventions.
• Excellent interpersonal and communication skills.
• Strong ability to advocate for patients and families within the healthcare team and beyond.
• Experience working with the terminally ill and their families, particularly in providing counseling and support.
• Empathy and compassion towards seriously ill patients and their families.

Qualifications

• Masters of Social Work from a Council on Social Work Education-accredited institution, or equivalent experience where licensure is required by state regulations.
• At least one year of social work experience in a healthcare setting.
• Experience in providing counseling and support to patients and families.
