Social Worker (SW)

Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Affirmatively ensure that VITAS patients have their affairs in order regarding finances, funeral arrangements, and other matters.
• Navigate the grieving process for patients' loved ones, helping them through their loss.
• Support patients in achieving their end-of-life wishes, whether large or small.

Requirements

• Reliable transportation.
• At least one year of social work experience in a healthcare setting.
• Thorough knowledge of community resources or the ability to acquire such knowledge.
• Knowledge of key psychosocial issues related to terminally ill patients and their families.
• Excellent listening and communication skills.
• Ability to advocate for patients and families within the patient/family unit, interdisciplinary team, and community agencies.
• Experience in providing counseling and support to patients and families.
• Empathy and compassion in dealing with seriously ill patients and their families.

Qualifications

• Masters of Social Work from a Council on Social Work Education-accredited school, or equivalent experience where allowable by state regulations.
• Licensure as per minimum state requirements in applicable states.
