About the Role
Provide Selfhelp Active Services for Aging Model (SHASAM) services and supports tailored to different stages in the aging process, available to residents upon request throughout their tenancy in Selfhelp Housing. Meet client needs for housing stability and reducing social isolation through an array of case management services.
Responsibilities
- Conduct intakes, provide accurate information, and make referrals.
- Screen and assist clients with applications and re-certifications for entitlements and benefits.
- Assess clients in their home, hospital, or other institutional settings as appropriate.
- Advocate on behalf of clients and arrange for provision of services; develop, implement, and follow up on care plans.
- Provide supportive counseling to clients as needed.
- Maintain ongoing communication with clients, family members, and other collaterals as appropriate.
- Maintain complete and accurate case notes using the program’s data system; prepare required documentation in case files within organizational time frames.
- Maintain and report statistics according to Selfhelp standards, utilizing the program’s data system and other record-keeping methods as needed.
- Actively participate in supervision and staff meetings; share emergent issues with supervisors in a timely manner.
- Facilitate and assist at social, recreational, and other group activities and events.
- Supervise social work students (if applicable).
- Adhere to the organization’s standards by completing annual mandatory trainings on topics including Sexual Harassment Prevention, IT Security Awareness, Compliance, OSHA, HIPAA, and other regulatory or business-related trainings.
- Support the organization’s mission by striving for excellence in all aspects of the job, focusing on positive interpersonal relationships with co-workers.
- Adhere to the organization’s policies regarding absenteeism, appearance, health, and safety standards.
Requirements
- Master’s Degree in Social Work or a related field.
- Working knowledge of Microsoft Office Suite and other technology.
- Excellent communication and listening skills.
- Excellent customer service skills with a focus on treating clients with respect and dignity.
- Able to multi-task and work independently with great attention to detail.
Pay
Salary range: $60,000 - $62,400 per year.
