About the role
The Social Worker at Mount Sinai West will assess and evaluate patients/families' social, psychosocial, and environmental situations to determine patient needs. They will demonstrate knowledge of the disease entity and common medical interventions in the service area assigned.
Responsibilities
- Afford options and treatment plans in a timely manner.
- Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals.
- Identify, select, access, and make referrals to appropriate community resources.
- Monitor, coordinate, manage activities for high-risk psychosocial patients to ensure implementation of multiple services.
- Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies.
- Identify and assist team in managing obstacles to treatment and/or discharge plans.
- Provide psycho-education for patients and families.
- Knowledge of child protection, elder abuse, domestic violence policies and protocols.
- Participate in interdisciplinary rounds, case conferences, staff meetings.
- Document in the medical record in accordance with standards set by Department of Social Work and/or regulatory agencies.
- May supervise social work interns.
Qualifications
- Masters in Social Work from an accredited school of social work.
- Knowledge of social work services, theories, techniques, community resources and agencies.
- Related experience in the social service and/or health care field, preferably one year in a medical setting, which can include related field placement.
- New York State Department of Education Licensure Licensed Masters of Social Work (LMSW).
