Social Worker - Social Work - Mount Sinai West - Full Time - Day

About the role

The Social Worker at Mount Sinai West will assess and evaluate patients/families' social, psychosocial, and environmental situations to determine patient needs. They will demonstrate knowledge of the disease entity and common medical interventions in the service area assigned.

Responsibilities

• Afford options and treatment plans in a timely manner.
• Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals.
• Identify, select, access, and make referrals to appropriate community resources.
• Monitor, coordinate, manage activities for high-risk psychosocial patients to ensure implementation of multiple services.
• Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies.
• Identify and assist team in managing obstacles to treatment and/or discharge plans.
• Provide psycho-education for patients and families.
• Knowledge of child protection, elder abuse, domestic violence policies and protocols.
• Participate in interdisciplinary rounds, case conferences, staff meetings.
• Document in the medical record in accordance with standards set by Department of Social Work and/or regulatory agencies.
• May supervise social work interns.

Qualifications

• Masters in Social Work from an accredited school of social work.
• Knowledge of social work services, theories, techniques, community resources and agencies.
• Related experience in the social service and/or health care field, preferably one year in a medical setting, which can include related field placement.
• New York State Department of Education Licensure Licensed Masters of Social Work (LMSW).
