Social Worker (PRN) - Hospice

About the role

We are seeking a compassionate and dedicated Licensed Social Worker to join our team in an outpatient hospice setting. The ideal candidate will have a strong background in geriatrics and hospice care, serving patients and their families in the Ridgeland and Vicksburg, MS areas, including surrounding and outlying communities.

As a key member of the Interdisciplinary Group (IDG), you will assess patient needs, develop individualized care plans, and ensure compliance with hospice care standards.

Responsibilities

• Conduct initial emotional, spiritual, and psychosocial assessments
• Provide ongoing counseling and support to hospice patients and their families
• Offer bereavement services
• Participate in community education, outreach, and referrals
