About the Role

The Care Management (CM) Services reports through the division of Patient Care Services and includes Case Management, Utilization Management, and Social Work departments. The Social Work department operates 24 hours/day, providing coverage throughout the hospital, including the Emergency Department, Outpatient Clinics, Registration sites, and Health Information Management.
Schedule
PRN position requiring a minimum of 2 shifts every 6 weeks, with a focus on weekday and weeknight coverage.
- Shifts: 7am-7pm (days), 7pm-7am (nights), midshift (3pm-11pm)
- Flexibility in hours is required; ability to fill partial or full shifts
Responsibilities
- Provides direct clinical services to patients and families in the Emergency Department, including coverage for all level 1 traumas.
- Handles crisis situations, traumas, and quick interventions, including death and dying scenarios.
- Addresses gunshot wounds and collaborates with police as needed.
- Manages concerns related to victims of violence.
- Provides interventions and resources to patients and families.
- Collaborates with physicians to deliver death notifications to family members.
- Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
- Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
Requirements
- Master's Degree in Social Work (MSW) – minimum requirement.
- Employees working at an Illinois facility must have an Illinois state social work license or be able to take and pass the licensing exam within 90 days of employment.
Preferred Qualifications
- 2-5 years of experience in social work.
- Licensed Master Social Worker (LMSW) certification.
