Social Worker, PRN

About the Role

The Case Manager Social Worker works in conjunction with the RN Case Manager to manage socially complex patients identified by initial assessment and referral. The Social Worker identifies the optimal resources available to meet the ongoing needs of the patients and their caregivers along the care continuum and is required to initiate this plan on admission.

Responsibilities

• Assist patients with complex psychosocial needs requiring eligibility determination for social programs and funding sources.
• Qualify patients for community assistance from a variety of special funds and agencies.
• Provide short-term counseling and crisis intervention to patients and families with psychosocial needs.
• Coordinate and facilitate the development of a safe discharge plan of care for high-risk patient populations.

Requirements

• Graduate from an accredited Social Work Program with a Bachelor’s in Social Work (required).
• Master’s in Social Work (MSW) (preferred).
• Clinical rotation experience may be considered in lieu of experience if applicable to the role.
• Experience in hospital social work (preferred).
• Knowledge of managed care practices, community resources, regulatory, and insurance reimbursement issues (preferred).

Position is PRN (as needed) with temporary standard hours at Erlanger East, Chattanooga, TN.
