About the role
This Social Worker position is in Social Work Service at the VA Healthcare Systems, located at the Jamaica Plain campus. This position is a full-time at 40 hours per week.Responsibilities
Major duties include, but are not limited to:- Conduct comprehensive psychosocial assessments and develop to address psychosocial concerns.
- Provide individual and clinical case management.
- Coordinate services with interdisciplinary treatment teams, VA programs, and community agencies to ensure continuity of care.
- Aid Veterans in addressing biological, psychological, social, and environmental needs to improve health outcomes.
- Serve as a consultant to physicians, nurses, and other health care providers by providing psychosocial expertise to support patient care.
- Inform Veterans and their families regarding VA benefits, community resources, and available support services.
- Advocate on behalf of Veterans to facilitate access to appropriate treatment and community resources.
- Maintain effective working relationships with Veterans, families, staff, and community partners while communicating effectively with individuals from diverse cultural and socioeconomic backgrounds.
- Develop and implement treatment plans for Veterans experiencing acute, chronic, or complex medical and mental health conditions.
- Facilitate care coordination for Veterans from all wartime eras, ensuring appropriate referrals and follow-up services.
- Participate in Whole Health initiatives, including facilitation of individual and group sessions, health coaching, and practice improvement activities.
- Complete clinical documentation in accordance with VA policies, Joint Commission standards, and professional social work practice requirements.
Requirements
- This position is eligible for the Education Debt Reduction Program (EDRP).

- This position is in the Excepted Service.
- Due to a critical staffing shortage, 38 U.S.C. § 7412 waives the requirement to apply Veterans' preference for this job announcement.
- The incumbent may be required to travel to other VA campuses and CBOC's.
Qualifications
- Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
- Must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
- May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
