SOCIAL WORKER - OUTPATIENT MENTAL HEALTH SERVICES (NDHC)

Essential Duties, Functions And Responsibilities

  • Provides mature professional judgment, utilizes treatment modalities, exercises culturally sensitive therapy, and performs crisis intervention.
  • Performs therapeutic skills in the evaluation, diagnosis, and treatment of patients and their families dealing with various health-related, psycho-social, and environmental issues.
  • Provides a continuum of care through community-based resources, integrates treatment plans for patients and their families, focusing on their recovery and return to health.
  • Prepares and interprets patient case studies and psychosocial histories for physicians, health team members, and community social services agencies.
  • Conducts initial assessments and evaluations of patients and families, identifies treatment needs and barriers to growth and development, and develops treatment plans addressing outcomes.
  • Collaborates with the medical team on psychosocial factors related to illness, hospitalization, diagnosis, and recommended treatments.
  • Coordinates services with community agencies and professionals such as social services, education counselors, and behavioral health personnel.
  • Initiates and continues community organization services to influence public attitudes towards patient needs and healthcare.
  • Collaborates and coordinates with tribal communities, courts, and community groups to develop necessary services and programs addressing unmet needs.
  • Makes professional decisions, planning, and recommendations for patient treatment plans, hospital discharges, and patient placement.
  • Completes required workload and provides periodic reports on the progress of medical social work and patient or community problems or unmet needs.
  • Gathers data through chart reviews and information required to evaluate the program and address health-related, psychosocial, and environmental issues.
  • Conducts limited performance improvement projects related to psychosocial unmet needs.
  • Participates in consultations with patients, families, community resources, and collateral staff.
  • Engages in program planning to enhance quality and comfort care for frail and vulnerable patient populations, fostering public understanding and sound working relationships with local agencies and community resources.
  • Attends conferences, community, group, and Tribal meetings relevant to Medical Social Work Services to improve community resources.

Mandatory Minimum Qualifications

  • Experience: Two (2) years of direct work experience.
  • Education: Master's Degree in Social Work.
  • Licensure: Valid and unrestricted license in any U.S. State in any of the following areas: Licensed Clinical Social Worker (LCSW), Licensed Independent Social Worker (LISW), License Independent Clinical Social Worker (LICSW).

Pursuant to this Policy, applicants who meet the minimum qualifications for this position and who are enrolled members of the Navajo Nation will be given primary preference in hiring and employment for this position, and members of other federally-recognized Indian tribes will be given secondary preference. Other candidates will be considered only after all candidates entitled to primary or secondary preference have been fully considered.
