About the role
The Social Worker helps patients and families adjust to illness and life-style changes due to illness through counseling and coordination of community, social and hospital services, and intervenes appropriately in cases of suspected domestic violence or abuse.
Responsibilities
- Provide counseling to patients and families
- Coordinate community, social, and hospital services
- Intervene in cases of suspected domestic violence or abuse
- Complete social assessments
- Address referrals
- Pre‑certify new patients
- Run family meetings
- Facilitate support groups
Requirements
- Master's Degree in Social Work
- General experience
- Relevant experience
- Required licensure
- Behavioral Health experience
- Utilization and health insurance knowledge
- Ability to work with patients 50 years and older
