Social Worker, MSW, PRN, Days

Responsibilities

• Promote patient and family wellness
• Improve care outcomes
• Facilitate access to appropriate hospital and community resources
• Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments

Qualifications

Education: Master's degree from an accredited social work program (Required)

Work Experience: No experience required; 2 years of experience in an acute or post-acute setting (Required)

Licenses and Certifications: LMSW - Licensed Medical Social Worker - State Licensure in the state of Georgia (Preferred)
