Social Worker MSW PRN

About the role

The Social Worker (MSW) at HCA Florida Northwest Hospital is responsible for non-clinical, biopsychosocial assessments and interventions for various inpatient and outpatient departments. The role involves crisis intervention, patient/family interventions, high-risk screening, emotional support, and referrals for financial assistance or other needs. The scope of the role includes intensive care units, trauma, oncology, NICU, transplant, and emergency department.

Responsibilities

• Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual barriers.
• Maintain a thorough working knowledge of Medicare, Medicaid, and private payer regulations and processes.
• Develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers.
• Support patients and families in making difficult decisions while honoring their preferences and values.
• Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
• Provide emotional support to patients and family members during complex and/or emotionally charged issues, such as end-of-life, terminal illness notifications, death notification (with physician), and crisis interventions.

Qualifications

• Masters of Social Work (MSW) required, with 1+ years’ experience or MSW internship preferred.
• Licensed Clinical Social Worker preferred.
• Certification in Social Work preferred.
• Critical thinking skills, service excellence, and good interpersonal skills.
• Ability to read and comprehend written/verbal instruction.
• PC Skills.
