Social Worker - MSW Part Time

About the role

Provides psychosocial services to dialysis patients, supporting their emotional, psychological, and behavioral well-being using social work principles. Collaborates with the healthcare team to promote positive adjustment, rehabilitation, and quality of life, while supporting quality indicators and improvement activities.

Responsibilities

• Assess patients' psychosocial status, develop care plans, and provide counseling and education tailored to individual needs.
• Assist patients with treatment adherence, transplant referrals, and accessing community resources such as vocational and educational programs.
• Support advance directives discussions and coordinate with the healthcare team on DNR and other critical decisions.
• Document interventions and participate in quality reviews, addressing non-adherence and patient satisfaction concerns.
• Collaborate with insurance coordinators to address coverage issues and provide financial education.
• Contribute to staff training, team meetings, and maintain updated patient resource information.

Qualifications

• Master's in Social Work with a valid state license.
• 0-2 years of related experience.

Skills

• Strong assessment, counseling, education, and collaboration skills; knowledge of community resources; adherence to policies and documentation standards.

Additional Details
