Social Worker MSW - Ochsner St. Anne - full time

About the Role

This position assumes responsibility for bio-psychosocial assessments, patient education, evaluation, referral to appropriate community resources, and supportive and crisis interventions. The role applies thorough knowledge and use of company and community-based services to coordinate and implement complex discharge plans. You will collaborate and communicate (verbally and through medical records) with the multidisciplinary team to evaluate and respond to patient and caregiver needs, including emotional, social, cultural, and spiritual needs related to adapting to the patient’s medical condition. Additionally, this role may practice clinical social work within the agency under the supervision of an LCSW employed by the company.

Responsibilities

• Conducts patient assessment interviews, provides referrals, and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records, documents, and communicates critical information to the medical team.
• Maintains professional and technical knowledge and required certifications through continuing education and development.
• Utilizes management resources and participates in on-call rotation.
• Adapts behavior to the specific patient population, including respect for privacy, method of introduction, adapting explanations, requesting permissions, and communication style.
• Remains knowledgeable on current federal, state, and local laws, accreditation standards, or regulatory agency requirements and ensures compliance.
• Other related duties as required.

Requirements

• Master’s degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE).
• Certified Social Worker (CSW) as verified by the LABSWE or Registered Social Worker (RSW) certificate.
• Preferred: Licensed Master Social Worker (LMSW) as verified by the LABSWE or must schedule and pass the LMSW exam within 6 months of employment.
• Preferred: Two years of experience in a medical setting.

Skills

• Computer skills and dexterity required for data entry and retrieval of job-related information.
• Proficiency with Windows-style applications, keyboard, and role-specific software packages.
• Effective verbal and written communication skills, with the ability to present information clearly and professionally to varying levels of individuals.
• Working knowledge of common disease entities, medical terminology, and the psychosocial and economic effects of illness/trauma on patients and their support systems.
• Knowledge of healthcare systems and public programs such as Medicare, Medicaid, Medicare Advantage, and commercial insurance plans.
• Strong interpersonal skills to deal effectively and courteously with patients, families, multidisciplinary team members, peers, and community agency personnel.

Physical and Environmental Demands

• Medium Work: Exerting 20 to 50 pounds of force occasionally, 10 to 25 pounds frequently, and negligible to 10 pounds constantly to move objects.
• Must be able to sit and stand for prolonged periods.
• Must be able to travel throughout and between facilities.
• Must be able to work a flexible schedule, including 24/7 availability, weekends, holidays, and on-call rotations.
• Normal routine involves no exposure to blood, body fluid, or tissue, but potential exposure may occur.
• Occupational risk for exposure to hazardous medications, hazardous waste, and communicable diseases within the healthcare setting.
