About the role
Aspirus At Home is seeking a SOCIAL WORKER – MSW to join our Hospice field team to cover sites within Northern WI (Eagle River, Woodruff, Tomahawk, Rhinelander).
Responsibilities
- Assists patients and families in coping with concerns related to end of life
- Works with patients and families to obtain psychosocial history
- Assists patients in resolving environmental difficulties interfering with attainment of maximum quality of life
- Conducts routine discharge planning
- Directs patients to designated community agencies or resources, as necessary
- Serves as a core interdisciplinary member who provides necessary social work services to patients/families under the direction of a physician
Schedule
Full Time or 1.0 FTE, 80 hours every pay period, 8-hour day shifts Monday – Friday
Qualifications
- Master's degree in Social Work from a social work curriculum accredited school or approved by the Council on Social Work Education
- Possesses Wisconsin or Michigan Certified Social Worker licensure required per the state in which you are practicing
- Minimum of one-year medical social work experience required
- Possesses and maintains a valid Wisconsin or Michigan Driver's License required
- Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required
- Demonstrates interpersonal skills to obtain information from patients and families, provide counseling to same, and interact effectively with patients, families and hospital personnel
- Displays analytical skills in order to assess patients' needs, develop associated discharge planning and provide sound advice and guidance
- Displays ability to concentrate and pay close attention to detail when interviewing patients, providing therapy, preparing discharge plans and the like
- Ability to cope with a work environment that deals with death, dying and bereavement
- Tolerates exposure to animals
Additional preferred qualifications
- Coursework in Medical Social Work beneficial
- Academic concentration regarding psychosocial aspects of terminal illness and bereavement, and education in death, dying, grief and loss beneficial
- Social work experience working with the terminally ill and on multidisciplinary teams preferred
- Previous professional experience working with Hospice, and in medical and home settings beneficial
